IN OFFICE DENTAL MEMBERSHIP

Alex Kalmanovich / Laguna Family Dentistry In-Office Membership is a discounted dental plan offered to individuals and their families. It is designed to help our patients without dental insurance, reduce the costs of maintaining their oral health. If you do not have dental insurance, for a low annual fee, our plan can provide you with preventive dentistry and a discount on all dental services performed in our office as described below:

PREVENTIVE SERVICES:  Covered 100% with no out-of-pocket cost.

• Two routine dental cleanings per year
• Two periodic examinations with oral cancer screening by dentist per year
• Two fluoride treatment per year – ***FOR CHILDEREN UNDER AGE OF 18 YEARS ONLY***
• All X-rays needed throughout the year, including full set of x-rays
• One emergency exam per year

RESTORATIVE SERVICES: 20% off our office fees on the following services:

• Crowns
• Bridges
• Dentures and Partials
• Fillings
• Sealants
• Periodontal Services
• Night Guards and Athletic Sports Mouth Guards
• Implants
• Laser

***DISCOUNTS DO NOT APPLY OR INCLUDE:

• Invisalign
• Cosmetic Procedures

COST OF MEMBERSHIP:

ADULTS:       From $579 to $779 per individual per enrollment year

CHILDREN:   $379 per dependent child (under age of 18) per enrollment year

ALL ENROLLEE:

• Services rendered and covered by this Plan must be paid in full at the time of service to be eligible for the discount. NO DISCOUNTS APPLIES IF NOT PAID IN FULL AT TIME OF SERVICE.
• CARE CREDIT financing is not a payment option.
• Treatment of any diagnosed needs not completed within 90 days forfeits any discounts.
• Scheduled appointments require a minimum of 48- hour cancellation notice to avoid $50 charge.
• If a hygiene appointment is failed, one of the covered, no charge, cleanings and exams is forfeited.
• The membership must be paid in full before any discounted services contained with the membership package are covered.
• Membership is for one year exactly of enrollment date.
• Memberships are NON- REFUNDABLE.
• Memberships are NON- TRANSFERABLE.
• No discounts on completed treatment or treatment in progress prior to purchasing membership.
• No discounts on services performed by Specialists who we refer when indicated.
• Patients covered by group or individually purchased dental insurance plans are ineligible.
• Discounts offered by this plan cannot be combined with other practice promotions.
• Plan benefits cannot be carrier over to subsequent years.
• Services included in the annual fee can only be used in the plan year in effect.

DISCLAIMER

Alex Kalmanovich, DDS / Laguna Family Dentistry shall be the sole party to determine participation eligibility, discounts, and eligible services rendered and covered under this Plan. The annual participation fee and discount percentages may change at our sole discretion.
